Spay Day 2010 Vouchers Available
until January 15, 2010

We are again offering Spay Day Vouchers to lock in a discounted Spay or Neuter for 2010.  These vouchers will be available until January 15, 2010 and must be purchased and signed in our office.

WE ARE NO LONGER ABLE TO ACCEPT CHECKS!  Besides cash, we can accept American Express, Discover, MasterCard, Visa, and ATM/Debit cards.

If you purchase a voucher, you may use it as a coupon for a half-priced microchipping of the same animal

when done at time of surgery.

The details of the voucher are below:

CVAH—Spay/Neuter 2010 Voucher for a Dog or Cat Under 7 years old

This voucher locks in discounted pricing for a 2010 spay or neuter.

The voucher covers a base price, any remainder must be paid before any and all products and/or services are rendered.

This voucher is non-refundable and void after 2010.  Voucher deposits cannot be applied elsewhere.

There will be no exceptions made to these guidelines.

~~~~~~~~~~~~CAT PRICING~~~~~~~~~~~~~

o I am purchasing a CAT Voucher for $30 which covers a male neuter.  If my cat is female, an additional $25 will be due at time of *surgery.  Spay pre-payments will be treated as a credit to my account.

*Barring complications (such as cryptorchidism, aphroditism, in-heat, or pregnant) this pricing for a routine surgery is locked in by the purchase of this voucher.  Any remaining balance or charges will be due in full prior to service.

~~~~~~~~~~~~DOG PRICING~~~~~~~~~~~~~

o I am purchasing a DOG Voucher for $55 which covers a male of 2-25lbs or a female of 2-20lbs.

Female Spay of 26-50lbs or Male Neuter of 21-50lbs:  an additional $10 will be due at time of *surgery.

Female Spay of 51-80lbs or Male Neuter of 51+lbs:  an additional $20 will be due at time of *surgery.

Female Spay of 80+lbs:  an additional $40 will be due at time of *surgery.

Any pre-payments will be treated as a credit to my account.

~~~~~~~~~~~~VOUCHER GUIDELINES~~~~~~~~~~~~~

o I understand that my pet must be deemed healthy (including dental health) on the day of surgery.  Pre-surgical bloodwork is strongly recommended for all pets.  If I am in doubt as to my pet's health, I should have it examined BEFORE purchasing a voucher.

o I understand that all pets brought in for surgery must be free of fleas and other external parasites to prevent surgical contamination and transfer to other hospitalized patients.  If my pet is found to have external parasites, it will be treated prior to surgery at my expense.  Parasite treatment is due when picking up.

o I understand that if my pet is not old enough to be completely vaccinated or if I have not kept my pet's vaccinations current, I am taking full responsibility for putting my pet at risk of contracting a contagious disease.

o I understand that no guarantee can ethically be made as to the results of any surgery and that there are always risks involved with any surgery, including but not limited to death or post-operative complications.  If I have questions about my pet's recovery from surgery, I should seek veterinary advice and/or attention immediately.  I agree to pay in full for services rendered including those deemed necessary for medical or surgical complications or unforeseen circumstances.

o I understand that if I do not provide accurate information allowing the attending veterinarian to contact me or my agent with any questions or concerns about my pet, the attending veterinarian will use his or her best judgment concerning the welfare and treatment of my pet.

o I agree not to abandon my pet.  If I do not pick up my pet within a reasonable amount of time, I will be held responsible for boarding, care, and/or euthanization of my pet as well as for any and all collection costs.

By Kentucky State law, all cats, dogs, and ferrets must have proof of a current rabies vaccination given by a licensed veterinarian.  If I cannot provide this proof, I must pay to have my pet vaccinated at CVAH.

Thank you for your interest in Smokey's amazing story.  If you need something for your own pet, or would like to know more about the fastest fork remover in Kentucky, please come back after visiting Smokey's site.  And PLEASE DON'T FEED YOUR PETS TABLE SCRAPS!  At least not on a regular basis.  Special BBQ scraps may be taken to the kitchen sink, scraped into separate bowls for each pet, and then taken to different areas of the yard to prevent future defective fork injuries.  :)
