Veterinarians are now required to obtain a signed request form or other waiver before releasing patient medical records. If you are changing veterinarians or if you would like to provide medical records to a new owner, you may print this form to request your pet's medical records:

Your current veterinarian will need this form to release your records to us.

However, Dr's. Smith and Crosslin will generally be available by appointment only.
